DO NOT GO HERE. My father WAS in this place November 2025 until April 2026 is having to pay for May. & THANK GOD WE HAD PEOPLE VISIT ALMOST DAILY (who will have their own reviews to give) but in those 5 months I saw all sorts of issues at this facility. TONS of staff turnover… says alot about the person in charge right there. I am going go be updating this with a novel of problems but for now….. they’re understanding & level of assistance is not what they say. My Dad’s medicine was mismanaged many times. Just recently I went and found a pill left inside his bed. (Not the first time either picture below). Found him on the floor the other week as well then I called in an aid, she tried to then 10:45am offfer to him his 8 am pill. I got more to write on the mismanagement of medice but I wanted to get something out asap before someone else signs up. My Dad is never going back after he was sent to the ER because he fell and then the wound never got proper care so he was sent back & is still in the hospital. They are still charging us for this next month of May because we couldn’t give our 30 days notice that my Dad was going to leave. Sorry we didn’t know he was going to develope an infection under their “asssisted care” and basically not be looked after properly since having this wound forcing us to find other proper 24/7 care. If your loved once needs true ASSISTANCE do not even consider this place.

Evergreen Cottages memory care is a social model. We provide assistance with daily care needs. Bathing, dressing, showering, toileting feeding and medication management, laundry and housekeeping.
We bring in the care components that require licensed professionals such as nurse practitioners, doctors or home health nurses. These professionals work for the residents well-being and provide wound care and other required services.
Their recommendations should be followed for optimum outcomes. Failure to follow may place the resident and indeed other residents in harms way.
Sometimes a resident may need to be sent to the hospital for emergency treatment and observation. We use our best judgement for the safety of a resident when making a decision to send them to a hospital for treatment.
Evergreen Cottages demands excellence from their caregivers and we continuously evaluate their performance, provide staff training and make adjustments as necessary for the residents best care. This may seem to the observer as high turnover, which in today's labor market is unavoidable.
Before a resident is accepted to live at Evergreen Cottages, a comprehensive admissions packet containing the terms of service are provided and acknowledged by the family. We refer you to the admissions and residency agreement contained in the packet. The business terms of admission, notice and and discharge are contained in the packet.

My name is Cèsar. I went to Jasmine cottages to go to my school community hours and upon arrival I felt so welcomed everybody there is so nice and the faith kept really clean. The resident look happy and there’s always some sort of entertainment going on. I really recommend this Memory care

This cottage is the most caring “home”! It is beautiful and so cozy. The Executive Director ensures all the residents get top notch care. Her involvement with the residents is quite comforting. The meals are delicious, healthy and homemade.
If I had to choose a place for myself, I would be there!

Food is slop: served cold, out of can or frozen package, everyone eats the same! They serve Plain rice, instant potatoes, beef stew from a can, soup from package, hard breads (when they serve it), no tea is ever served. Nor is decaffeinated coffee served. Never. The posted menu is RARELY what is served. Remember, dementia residents can’t communicate what they do not like or do not want. They are given water & sometimes juice. It is common to only have a small bowl of soup several times a week for dinner. Night after night. That’s it.
The same employees who clean & shower incontinent patients, do laundry, clean rooms & toilets, and take out trash - also prepare & serve patient meals! CONTAMINATION.
Few employees speak English. These same employees dispense meds & do other medical tasks. This is frustrating for residents. Employees are understaffed & overwhelmed by what needs to be done during their shift. It is common to hear Staff yell at each other & residents.
In conclusion: ask for documentation of all advertised licenses, certifications, accreditation of dietitian & staff, proof of activities AND coordinator.
Proof of designated nurse(s), doctor, & when they are on campus. Proof of state inspections.
Lastly, unexpected visits are a must. Eat what is served. Know what your loved one is living with.

Evergreen Cottages is a Non Medical Social Community. We provide State Licensed residential board and care for people with Alzheimer's and other Dementia's.
Our Cottage is headed by an Executive Director who is a Nurse. She is onsite everyday and available for emergencies 24 hours a day, 365 days a year, except when off sick or on vacation, at which time our other cottage director next door is available along with our Registered Nurse and Operations Manager.
Note: State regulations do not require a nurse to be in the cottage at all times. Our Cottages have nurses available onsite. State staffing ratio of 1 caregiver to 8 residents is always maintained and exceeded.
For ease of communication, all resident families are provided with the directors Cel numbers and email addresses along with Policies and Procedures contained in the resident handbook. We note that the
We understand that families have to negotiate many rapidly changing conditions with their loved one upon diagnosis. We are here to help navigate the challenges.
Our Caregivers are " Universal Workers" as described in our resident handbook, a copy of which is provided with the registration packet. This model of staffing allows for all staff to be available to all residents at all times, regardless of the tasks being completed, including Housekeeping and Activities.
Our duty assignments are done in a manner that allows Kitchen staff to avoid contact with residents with incontinence, thus avoiding contamination. The care staff are fully trained in best sanitation standards and do not handle food without maintaining proper hygiene.
Our menus are built to provide nourishment and meals are served hot. The menu allows for flexibility to meet residents preferences. Deviation is often necessary, and alternatives always available for residents who may not prefer the menu items. This is the flexibility a small 16 bed community can provide. Unfortunately, some residents resist eating their meals on time, so staff spends time to encourage them to come to the dining table.
All licenses and certifications are displayed on the wall in the hallway across from the Kitchen. If you need any specific information, we will be glad to provide it. Please contact the Cottage Director. Note we are not a Medical Model and do not have Doctors on staff. Every resident has their own doctor. Nurse Practitioners, podiatrists, mobile dentists and other rehab and specialty services are brought in as prescribed by the residents doctors.
Our Moto at Evergreen Cottages is " We Work Where They Live" It's "grandpa and grandma's home" so the home is always open to visitors and we absolutely encourage families and friends to drop by unannounced and have a meal with us.
Family and friends are encouraged to volunteer in the community with everything from Activities, reading, kitchen help or music and entertainment. This is a community that encourages participation, engagement and most importantly " Communication" with staff and Directors. It takes a Village and we're all in this together.
