Kailyn just reached out to us on our family members behalf. She was sweet professional, and you could totally tell that she went out of her way to make our family member feel special and cared for and that really means a lot just off of one conversation. I hope that if anyone has to go through hard situations with family members that they get the same compassion that we have gotten from life choice today.
I cannot speak highly enough about LifeChoice Hospice and Palliative Care. I witnessed first hand what loving, patient, and empathetic quality Hospice care looks like. I give myself credit for choosing them, after a careful search.
Caring for my lifelong friend by myself was quite a challenge for me, physically; however her devoted nurse case manager, Mark, saw me through it with the help of his caring and compassionate team.
Thank you to nurse Dan, on-call nurse Grace, (who talked me “off the ledge” for a solid hour on the phone one scary evening,) social worker, Kailynn, CNA Yaritza, (and Nadia) spiritual counselor, Doug, Dr. Kaplan, and to the 24 hr LifeChoice staff, who fielded my many many calls asking for help, always with patience and problem resolution at hand.
You will not go wrong choosing LifeChoice.

I am very grateful to Life Choice for taking care of my wife during her last days with us. They provided everything she needed for her comfort. They were so responsive with phone calls 24/7. All the staff were so kind especially Nurse Bertha Cruz-Reyes who gave extra time to visit and make sure that everything is taken care of. Thank you Life Choice. My name is Shannon Parson Johnson, My father William parson was able to pass away with dignity and respect, I cannot say enough about the incredible care and compassion this hospice team provided for my father. Their whole demeanor and attitude is that of angels, compassion, empathy, love..When he was in the hospital, he was not receiving the proper pain management, they kept delaying the hospitals hospice program while my dad was dying in pain. I was desperate to get him home.
Alena, the administrator, moved mountains to make that happen. They got my dad home in less than 18 hours, they were by my father‘s bedside within two hours of me calling desperate for help, which felt like a miracle during such a difficult time.
Daniel, the nurse who came to the hospital, was so attentive and caring such a genuine person. And once we were home, nurse Julie and nurse Grace took over and truly went above and beyond. They made sure the bed oxygen tanks supplies and meds was delivered before my dad even got home.They made sure my dad was comfortable, out of pain, and surrounded by dignity and respect—something he was not receiving in the hospital. there was also a Sweet CNA that came to help also Cecilia
Because of them, my dad was able to pass peacefully at home, just as he deserved. That means everything to me my father and our family. All of my father‘s friends and loyal family were able to come visit with him before he passed the next morning..
I highly recommend this hospice agency to anyone going through end-of-life care for a loved one—or even for themselves. Don’t wait until the last minute. Have this hospice team set up ahead of time—you will not regret it.

They were absolutely wonderful the doctors staff were awesome . They were there when our family mostly needed them I'm so humbly grateful they took care of everything and gave us peace of mind. Our mom was very sick, we were so scared and sad when it was time for hospice. We are not from the area which made things even worse. We hated seeing her in the hospital—it felt so cold and unwelcoming. LifeChoice came to her house right away and brought warmth and experience. The nurses were sweet, personable and treated her like they were family. What we thought would be just a few days turned into weeks spending time together. They kept her comfortable the whole time, answered our calls 24/7, brought everything she needed and honestly were so kind to us that we really felt like we were not in it alone. She got to stay in her own bed surrounded by the people who love her most. The extra time allowed everyone to come and visit. LifeChoice turned the hardest days—and weeks—of our lives into something beautiful and full of love. 10/10 would recommend to anyone living in the area.

Losing both patents on hospice was very hard. We were blessed to have Rabbi Doug as our chaplain who brought us a lot of comfort and support.

I want to thank Bertha, our hospice nurse, from the bottom of my heart. She naturally gave my mom such warm and compassionate care. Bertha was also so resourceful and helpful with my husband and me, and even with my mom’s caregiver. She went above and beyond for us, and we will be forever grateful to her.

LIFECHOICE Hospice was amazing. They walked with us each step of the way the last few weeks of my Dad’s life. We felt fully supported and my Dad felt truly cared for. They were very responsive with phone calls, provided medications & supplies, made regular visits to our home and gave personal, caring attention. Even the young men who delivered medical supplies were friendly and helpful. We are especially grateful for Marina our nurse, Jessica, Carlene, Dan and Grace. Thank you LIFECHOICE for bringing compassion and dignity to my Dad’s passing.

Choosing a hospice care provider is an incredibly important decision & I am so grateful that we went forward with the team at LifeChoice. From top to bottom, my family & I were absolutely impressed with their attentiveness to address any questions or concerns we had throughout the process, and never felt any form of pressure regarding any decisions made.
We had the pleasure of working with Alina, Elif, Angel, Doug, & Alex and my family could not have asked for a better team of talented and caring individuals. Alina stayed on top of the onboarding process, ensuring no equipment was delayed during the initial start up, and always available when he had a question on how things worked. Alex & Angel regularly stopped by to verify that we had all of the supplies & medication we needed, as well as checking my mother's vitals and addressing any of our concerns. Doug was great about reaching out to check in with us, as well as verified our religious beliefs in case we were interested in any forms of service & guidance. Elif consistently checked in with myself & our family, always there to lend an ear and speak with each family member offering kind words and advice.
Never once did any of them make us feel like their duties were done to simply check a box. Every action, decision, and conversation came from a place of goodness and with all of us in mind. Even now, after my mother's passing, Elif has been kind enough to continue to reach out & check in with myself and my family to ensure we are managing okay.
Thank you all so much for all of your compassion & professionalism! It was an absolute delight working with LifeChoice and this team during this difficult time & I would highly recommend them to anyone who is looking into hospice care. You will not be disappointed.
