I would like to share our experience with Dr. Shashikant Gupta, Senior Urologist at Apollo Medics Hospital, Lucknow, regarding the treatment of my father Mr. Aishi Lal, 77 years old. We consulted Dr. Gupta sir when my father presented with anuria (complete stoppage of urine), high-grade fever, and significantly elevated serum creatinine levels, indicating severe kidney dysfunction. Dr. Shashikant Gupta conducted a thorough clinical examination and advised essential investigations. The CT Abdomen revealed left bulky kidney with bilateral hydronephrosis (B/L HDN), perinephric fat stranding suggestive of pyelonephritis, minimal ascites, cholelithiasis, and prostatomegaly. A detailed evaluation confirmed the diagnosis of Carcinoma of the Urinary Bladder with prostatic extension, complicated by bilateral acute pyelonephritis, and associated dementia with behavioral and psychological symptoms (BPSD) with overlapping delirium. Despite the high-risk nature of the case—due to renal failure, infection, advanced age, and multiple comorbidities—Dr. Gupta ensured timely interventions: 1. He stabilized the patient by managing the acute infection and renal dysfunction, correcting anemia, and optimizing overall health before surgery. 2. He performed a complex and high-risk Open Radical Cystoprostatectomy with Bilateral Retroperitoneal Pelvic Lymphadenectomy and Ileal Conduit Formation under general anesthesia. The surgical procedure was challenging due to the involvement of both ureters and the patient’s frail condition, but Dr. Gupta's expertise made it successful. The post-operative recovery was remarkably smooth, and my father could start oral intake within 24 hours after surgery.
