💸 APPOINTMENT FEES
- $350–$450 for a new patient evaluation
- $175–$200 for follow‑up visits
- $150 fee for canceling or rescheduling within 48 hours
- $150 fee for new‑patient no‑shows
- $75 fee for follow‑up no‑shows
- $75 fee for follow‑up cancellations under 48 hours
- If you’re 10 minutes late, they cancel your appointment and charge you anyway

💸 PAPERWORK FEES
- $75–$150 for ANY paperwork
- FMLA, ADA, Disability, Social Security paperwork not done until your 3rd appointment
- 10–14 business days for paperwork
- You must pay upfront before they even start it

- $30 for a prescription refill before your appointment
- $30 for any refill if you missed or canceled an appointment
- $75 for medication overrides
- $30 for a second prior authorization
- No refills processed after 12 noon on Fridays
- Controlled meds can only be moved to a different pharmacy once
